Job Description
Physical Therapist Assistant (PTA)
Put People First. Do Work That Matters.
At Haven Home Health & Hospice, we believe care should always be about the person, not the profit.
As a locally owned organization, we have the opportunity to make decisions with our clients, their families, and our community in mind. We are looking for a Physical Therapist Assistant who shares that mindset and wants to make a meaningful difference in the lives of the people they serve.
As a PTA at Haven, you will work alongside a collaborative team of therapists and clinicians to provide skilled physical therapy services that help clients remain safe, independent, and active in their own homes. You will build relationships with clients and families while helping them work toward the goals outlined in their individualized plan of care.
What You’ll Do
- Provide skilled physical therapy services under the supervision of a Physical Therapist.
- Implement individualized plans of care based on physician orders and client needs.
- Monitor client progress and communicate changes with the supervising PT and care team.
- Provide preventive and rehabilitative therapy interventions.
- Educate clients and families on self-management, safety, and physical therapy needs.
- Maintain accurate and timely clinical documentation.
- Collaborate with other Haven clinicians to provide coordinated, client-centered care.
- Participate in department meetings, in-services, clinical outcomes, and performance improvement initiatives.
- Uphold Haven policies, professional standards, and applicable regulatory requirements.
- What We’re Looking For
- Current Missouri PTA license in good standing
- At least one year of PTA experience within the last three years
- Strong communication and organizational skills
- Ability to build meaningful relationships with clients, families, and fellow team members
- Strong clinical judgment and ability to work independently
- Comfortable with technology and electronic documentation
- Dependable transportation and a current driver’s license
- Flexibility to work weekends and holidays as needed
Why Haven?
At Haven, people come first. As a locally owned organization, we are personally invested in the clients and communities we serve. We believe in honest communication, accountability, doing what is right, working together, and continuing to grow both personally and professionally.
The care you provide can help someone regain strength, maintain independence, and continue living life on their terms. That is the kind of work that matters.
If you are a Physical Therapist Assistant who wants to build meaningful relationships, be part of a supportive team, and provide care with purpose, we would love to have you join Haven Home Health & Hospice.
