Key Responsibilities
Perform comprehensive health assessments and manage acute and chronic conditions for residents in senior living communities. Collaborate with interdisciplinary teams to coordinate care plans and improve patient outcomes while reducing hospitalizations.
Requirements
Requires a valid nurse practitioner or physician assistant license and current board certification in the field. Must hold a Master's degree in Nursing or Physician Assistant Studies and be capable of traveling to assigned facilities.
