Qualifications

* DDS or DMD from an accredited dental school

* Active Oregon dental license (or ability to obtain prior to start)

* Current DEA registration, CPR certification, and NPI (or ability to obtain)

• Oregon Office of Rural Health (ORH) and the National Health Service Corps
• (NHSC). Qualified providers may be eligible for up to $150,000 in student loan
