Key Responsibilities

The ambulatory clinical pharmacist ensures safe, effective, cost-effective, and high-quality medication therapy disease management for patients in the ambulatory setting, working under a collaborative practice agreement with physicians. Responsibilities include direct patient care via in-person or telephone interventions, consulting with patients and physicians to resolve medication-related problems, and managing patient medication therapy to optimize clinical outcomes.

Requirements

Candidates must possess a Bachelor's Degree in Pharmacy and must obtain a Licensed Pharmacist certification for the State of Pennsylvania within 90 days. The role requires proficiency in medication and disease management, the ability to gather necessary clinical data, and skills in teamwork to provide high-quality medication disease management services.
