The Clinical Pharmacist will lead medication optimization for chronic cardiometabolic conditions by analyzing utilization data and designing interventions to improve adherence and reduce costs. They will collaborate with multidisciplinary teams to provide medication therapy management and ensure rigorous documentation within virtual care workflows.
Candidates must hold a PharmD degree from an ACPE-accredited college and have completed an ASHP-accredited PGY-1 residency. A minimum of 3 years of clinical pharmacy experience in ambulatory or value-based care is required, along with an active, unrestricted pharmacy license.
