Key Responsibilities
Conduct comprehensive preventive health assessments during in-home visits to identify care needs and close gaps. Connect members to appropriate ongoing care while maintaining high-quality, autonomous patient interactions.
Requirements
Must hold an active Oregon and/or Washington Nurse Practitioner license and be nationally board certified. Clinical experience in family medicine, primary care, internal medicine, urgent care, or geriatrics is preferred.
