Community Home Health Care is hiring licensed Physical Therapists and Occupational Therapists for part-time and PRN home visits in the Fort Wayne area.

In home-based care, therapy is personal. You work one-on-one with each client in the place where they need to use their skills every day. Your visit can help someone move more safely, manage daily activities, reduce fall risks, and remain as independent as possible at home.

  • Evaluating mobility, gait, strength, balance, transfers, and fall risk
  • Developing treatment plans around the client’s goals and home environment
  • Providing exercises and functional mobility training
  • Teaching safe transfers and use of mobility equipment
  • Identifying home safety concerns and recommending practical changes
  • Showing clients and caregivers how to support progress between visits

Occupational Therapist (OT)

Help clients take part in everyday activities as safely and independently as possible. Work may include:

  • Evaluating daily activities such as dressing, bathing, grooming, and meal preparation
  • Building skills and routines that support greater independence
  • Teaching energy conservation, safe movement, and adaptive techniques
  • Recommending adaptive equipment or changes to improve home safety
  • Supporting upper-body function, coordination, and functional reach
  • Educating clients and caregivers on strategies they can use between visits

We’re Looking For

  • Current Indiana license as a PT or OT
  • Compassionate, dependable clinicians who enjoy one-on-one care
  • Strong communication, organization, and documentation skills
  • Confidence working independently in clients’ homes and communicating with a care team
  • Home health experience is helpful

Schedule and Pay

  • Physical Therapist: $50–$60 per hour
  • Occupational Therapist: $47–$57 per hour
