Key Responsibilities
The Clinical MTM Pharmacist provides patient-centered medication therapy management services to optimize clinical and economic outcomes. They conduct comprehensive medication reviews, targeted interventions, and collaborate with interdisciplinary teams to improve patient health.
Requirements
Candidates must hold a Bachelor's degree in Pharmacy or a PharmD and possess an active pharmacist license. Strong clinical knowledge, MTM certification, and excellent telephonic communication skills are required for this role.
