Key Responsibilities

The social worker is responsible for assessing patients, facilitating discharge planning, and intervening in social, emotional, and economic issues to ensure a smooth transition of care. They also provide leadership by mentoring new staff, supervising LSW team members, and coordinating continuing education initiatives.

Requirements

Candidates must hold a Master's degree in Social Work and possess either an LCSW or LSW license in Illinois. At least one year of technical experience in a healthcare or social work setting is required.
