Perform ongoing clinical assessments, including OASIS, and develop individualized plans of care for geriatric patients in their homes. Provide skilled physical therapy treatments, supervise therapy assistants, and maintain accurate clinical documentation.
Requires a current Physical Therapist license and strong clinical assessment skills. Previous experience in home health or a related healthcare setting is preferred.
