RN Case Manager - Hospice

Key Responsibilities

The RN Case Manager coordinates comprehensive, patient-centered end-of-life care and serves as the primary clinical point of contact for patients and families. They are responsible for conducting nursing assessments, developing individualized care plans, and collaborating with the interdisciplinary group to ensure continuity of care.

Requirements

Candidates must hold a current, active, and unrestricted RN license with at least 2 years of nursing experience, including 1 year in hospice, palliative care, or a related field. Strong clinical assessment skills, computer proficiency, and a valid driver's license with reliable transportation are required.
