The Case Manager conducts face-to-face home visits to perform comprehensive assessments and develop care plans for members in managed long-term services programs. They coordinate care with interdisciplinary teams and providers to ensure cost-effective, safe, and high-quality health service delivery.
Candidates must possess an active and unrestricted RN license in New Jersey and at least 3 years of clinical practice experience. Successful completion of the NJ Choice certification is a mandatory contingency of employment.
