Consultant Pharmacist (PACE Program)
Mt. Pleasant, MI
Full-Time | Weekdays
$130K-$150K

Overview
We are seeking a compassionate and clinically strong Consultant Pharmacist to join our team in supporting a PACE (Program of All-Inclusive Care for the Elderly) program in Mt. Pleasant, MI. This role is ideal for a pharmacist passionate about geriatric care and interdisciplinary, patient-centered practice.

As a key member of the care team, you will help optimize medication therapy and improve health outcomes for older adults with complex medical needs.

What You'll Do
Review and manage participant medication regimens
Participate in interdisciplinary team (IDT) meetings and care planning
Conduct medication reconciliations and chart reviews
Identify and resolve medication-related issues
Ensure compliance with CMS, state pharmacy laws, and PACE guidelines
Educate participants, caregivers, and care team members

What We're Looking For

PharmD from an accredited school Current Pharmacist and Controlled Substance Licenses 2+ years of experience in geriatrics, LTC, or similar setting PACE experience and BCGP or Consultant Pharmacist certification preferred Willingness to travel

Key Responsibilities

The Consultant Pharmacist will manage participant medication regimens and conduct medication reconciliations within an interdisciplinary team. They are responsible for ensuring compliance with CMS and state pharmacy laws while providing education to participants and caregivers.

Requirements

Candidates must hold a PharmD degree along with current Pharmacist and Controlled Substance licenses. A minimum of 2 years of experience in geriatrics or long-term care is required, with preference given to those holding BCGP or Consultant Pharmacist certification.
