Key Responsibilities
The Social Worker will provide compassionate counseling and psychosocial support to hospice patients and their families. They will collaborate with the interdisciplinary team to develop and implement individualized care plans that address medical, emotional, and financial needs.
Requirements
Candidates must hold a Master of Social Work degree and a current state license as a Social Worker. At least one year of experience in a healthcare, medical-surgical, or hospice setting is required.
