Key Responsibilities
The Nurse Practitioner will conduct comprehensive in-home assessments and manage acute or chronic medical conditions for high-risk patients. They will collaborate with an interdisciplinary team to provide integrated care and ensure accurate clinical documentation.
Requirements
Candidates must hold an active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care. A valid driver's license, reliable transportation, and an unrestricted state license are also required.
