Key Responsibilities
The social worker provides professional care coordination and mental health support to patients and families within the TCHATT program. They conduct comprehensive assessments, develop individualized care plans, and facilitate referrals to community resources while collaborating with a multidisciplinary team.
Requirements
Candidates must hold a Bachelor's degree in Social Work or a related field and possess a Texas LBSW license, with LMSW preferred. A minimum of one year of social work experience is required, and candidates must reside in the Greater Houston area.
