Key Responsibilities
The Medical Social Worker will provide home-based social services, including patient assessments, counseling, and crisis intervention for hospice patients. They will collaborate with an interdisciplinary team to monitor psychosocial conditions and ensure patients meet their individual care goals.
Requirements
Candidates must hold a Master's degree in Social Work and an active MSW license in Kansas. Additionally, a CPR certification and strong interpersonal skills are required for this role.
