Key Responsibilities
The Case Manager coordinates patient-centered care and service delivery to facilitate optimal transitions and progression from pre-admission through post-discharge. They collaborate with the interdisciplinary team to manage clinical resources, identify barriers to care, and develop safe, cost-effective transition plans.
Requirements
Candidates must hold an Associate Degree in Nursing and possess 1-3 years of clinical healthcare experience in an acute or hospital setting. A valid Registered Nurse license in Tennessee is required, with proficiency in EMR systems and strong collaborative skills.
