Key Responsibilities
The Medical Social Worker provides psychosocial assessments, diagnostic evaluations, and patient-centered interventions as part of an interdisciplinary hospice team. They coordinate care, facilitate community resource linkages, and assist families with advance care planning and end-of-life decisions.
Requirements
Candidates must hold a Master's degree in Social Work and possess a valid South Carolina Social Work license (LMSW or LISW-CP). At least one year of health care experience is required, with hospice or home health experience preferred.
