Key Responsibilities
The Social Worker will assess the social, emotional, and cognitive needs of patients and their families to facilitate effective care planning. They will coordinate with community resources and maintain accurate documentation for all treatment plans and evaluations.
Requirements
Candidates must hold a Master's Degree in Social Work and possess the appropriate state licensure or be eligible for supervised practice. Experience in a hospital or acute care setting is required for this role.
