Key Responsibilities
The RN Case Manager provides comprehensive hospice nursing care by assessing patient needs, developing individualized care plans, and managing symptoms. They also supervise LPNs and hospice aides while collaborating with the interdisciplinary team to ensure high-quality, compassionate care.
Requirements
Candidates must have at least one year of professional nursing experience and a valid, unencumbered Registered Nurse license in Virginia. A valid driver's license, current CPR certification, and proof of flu vaccination are also required.
