The Inpatient Case Manager RN facilitates comprehensive care management and quality outcomes for patients across the healthcare continuum. They are responsible for managing patient transitions, monitoring health services, and developing care plans based on physical, mental, and psychosocial needs.
Candidates must hold a current Registered Nurse (RN) license in VA, NC, or a compact state and possess at least one year of nursing experience. A Bachelor's degree in Nursing is preferred, and certification in Case Management is required within one year of eligibility.
