Hamilton Health Box is a rapidly growing healthcare startup operating hybrid telemedicine-enabled primary care clinics for small and medium businesses across the country. Our goal is to find high potential individuals to drive a high-touch, low-cost healthcare experience for all of our patients and clients.
Position Summary
The Advanced Practice Provider (APP) will work closely with supervising Physicians and will manage 3-5 separate primary care micro clinics and care for the surrounding communities. It will consist of a combination of virtual and in-person medical care. The key objective is to create value through health coaching, disease management, preventative screening, and education.
Essential functions
- Promotes and maintains patient health by providing medical services under the supervision of a physician.
- Contributes to a physician’s efficiency by identifying short-term and long-range patient care issues.
- Recommends options and courses of action for patient care.
- Implements physician directives.
Assesses patient health by interviewing patients and performing physical examinations including obtaining, updating, and studying medical histories.
Determines abnormal conditions by administering or ordering diagnostic tests, such as X-rays and laboratory studies and interpreting test results.
Documents patient care services by charting in patient and department records.
Performs therapeutic procedures by administering injections and immunizations, suturing, and managing wounds and infections.
Instructs and counsels patients by providing normal growth and development information and providing counseling on emotional problems of daily living.
- Provides a community of care by developing and implementing patient-management plans.
- Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
Protects patients and employees by adhering to infection-control policies and protocols. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; and enforcing adherence to requirements and advising management on needed actions
Collaborates in the development and evaluation of patient care. Updates clinical guidelines/protocols/pathways; works autonomously and accepts accountability for outcomes; signs orders in accordance with institution and state board requirements; collaborates in research and teaching projects and applies research to practice.
Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
Develops health care team staff by providing information, educational opportunities, and experimental growth opportunities.
- Utilizes telemedicine interface to complete virtual visits.
- Ability to float, round to multiple facilities and/or take call (first call) to support Urgent Care APPs.

Requirements
Physician Assistant
Education: Graduate of a nationally accredited Physician Assistant program; Master of Physician Assistant Studies.
Licenses/Certifications
- Certified by the National Commission on Certification of Physician Assistants, Inc.
- Licensed by the New Mexico State Board of Physician Assistant Examiners or other identified state(s).
- Eligibility for DEA licensure.
- Basic Cardiac Life Certification.
