FeldCare Connects, an app-based referral network, is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Tracy and the surrounding areas.

Deliver quality home healthcare services to patients of all ages by observing, recording, and reporting on their condition. Develop and implement individualized plans of care while maintaining accurate documentation through an electronic medical record system.
Must hold a current, active, and unrestricted Physical Therapist license issued by the state Board of Physical Therapy. Requires completion of an accredited Physical Therapy program, reliable personal transportation, and a smartphone.
