Registered Nurse (RN) Case Manager

Arrange for necessary medical equipment and supplies. Ensures HIPAA compliance and patient confidentiality. Additional Duties Participates in on-call visits as required including evenings and weekends. Attend interdisciplinary group meetings. Supervises ancillary personnel and delegates responsibilities as needed. Engage in professional development through training and continuing education. Contributes to quality assessment and performance improvement activities. Performs other duties as assigned.

Qualifications

Requirements/Qualifications Must have a current Registered Nurse (RN) license in state(s) of practice. Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact. Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice. Ability to pass DHS background, consumer report, and drug test. Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required. Preferred 1+ years of recent experience in hospice/home care preferred. 2+ years of Nursing experience, preferred Physical Requirements The physical requirements described are representative of those that must be met to successfully perform the essential responsibilities of this position.
