Key Responsibilities
The social worker provides medical social casework to patients ranging from newborns to geriatrics to ensure optimal psychosocial functioning and continuity of care. They also collaborate with multidisciplinary teams to assist patients with medical recommendations, resource navigation, and discharge planning.
Requirements
Candidates must hold a Bachelor's degree in Social Work and possess an active social work license in the state of practice. Previous healthcare social work experience of 1-3 years is preferred.
