Key Responsibilities
The Physical Therapist evaluates patients in their homes, develops and implements treatment plans, and performs comprehensive assessments including OASIS. They also coordinate care with other medical professionals and educate patients and caregivers on home exercise programs and safety.
Requirements
Candidates must hold a Bachelor's degree in Physical Therapy and be licensed in Illinois. Additionally, they are required to have a valid driver's license, proof of auto insurance, and Basic Life Support (BLS) certification.
