Home Health Physical Therapist

Key Responsibilities

The Physical Therapist will perform home visits to provide rehabilitative care, including diagnostic testing, gait training, and individualized care plan development. They are also responsible for educating clients and families while accurately documenting care using electronic medical records.

Requirements

Candidates must hold a current VA Physical Therapist license and have graduated from an accredited physical therapy program. Strong organizational, interpersonal, and computer skills are required to manage independent home health visits.
