- This position is Monday through Friday day shift, 9:00 am- 5:00 pm (hybrid in person/virtual possible).

Key Responsibilities
The Social Work Resource Liaison will support the Outpatient Psychiatry Evaluation and Navigation (OPEN) Hub by reviewing and completing referrals, interfacing directly with patients, coordinating evaluations, and collaborating with team members in a fast-paced environment. This role involves participating in daily triage meetings and supporting patient disposition planning by finding outside resource information and responding to inquiries about community services.
