Join NewYork-Presbyterian Hospital as a Social Work Resource Liaison – M-F Day Shift in United States.

Key Responsibilities

• will be a tri-campus hub for triage, assessment, and evaluations to assist adult patients finding the right mental health care at the right time.

• will support our strategic initiative to improve access to our outpatient programs, provide transparency to available services and treatment within our system, make rapid and appropriate recommendations for services to a larger outside ecosystem while supporting Ambulatory Services’ mission to provide tertiary care to the most acute patients.

• will help support Outpatient Psychiatry Evaluation and Navigation (OPEN) Hub in delivering high quality patient care by reviewing and completing referrals, interfacing directly with patients, coordinating evaluations, and collaborating with various team members in a fast-paced, evaluation-focused environment.

• will participate in interdisciplinary daily triage meetings and support patient disposition planning by locating outside resource information and responding to inquiries about a broad range of community services, eligibility requirements and application procedures.

Requirements

• time day shift position at New York

• Presbyterian, physically based at Westchester Behavioral Health. This position is Monday through Friday day shift, 9:00 am

Benefits & Compensation

• Salary: $35.55 - $40.55/hr

• PSLF eligible (Public Service Loan Forgiveness).

Work Schedule

• Full-Time position.

• Hybrid position - combination of remote and on-site.

• Day shift only - no nights.
