Key Responsibilities
The Registered Nurse Case Manager provides holistic, individualized care to patients in their homes and leads end-of-life care planning. They are responsible for symptom management, administering treatments, and educating patients and families throughout the care process.
Requirements
Candidates must hold an accredited nursing degree and possess at least one year of recent acute care experience. A valid driver's license, reliable transportation, and proof of auto insurance are required for the 100% travel component of the role.
