Join our growing team! Seva Home Health is locally owned and growing company seeking an RN Case Manager to cover Madera and Fresno Counties. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. Home health experience ideal, and we will provide training and support.

Key Responsibilities
The RN Case Manager is responsible for initiating, implementing, and revising patient Plans of Care while delivering quality patient-centered care in home settings. They must coordinate with physicians and families to evaluate patient status and ensure accurate, timely documentation.
Requirements
Candidates must be a Registered Nurse with at least one year of experience, a valid license, and current CPR/BLS certification. Reliable transportation is required, and experience with Kinnser EMR or palliative care is strongly preferred.
