Key Responsibilities
Provide evaluative, consultative, and treatment interventions to home health patients in their place of residence. Collaborate with interdisciplinary teams and physicians to develop and manage patient care plans while maintaining accurate clinical documentation.
Requirements
Must hold a valid professional physical therapy license in the state of practice and be a graduate of an accredited Physical Therapy Program. Prior home health experience is preferred.
