Key Responsibilities
The RN Case Manager is responsible for processing palliative care referrals and coordinating patient care goals across providers. They will conduct home visits, manage documentation, and serve as a liaison for medical directors and nurse practitioners.
Requirements
Candidates must hold an active Registered Nurse or Licensed Practical Nurse license in the state of practice. A minimum of three years of nursing experience is required, with preference given to those with hospice or palliative care backgrounds.
