Key Responsibilities
The Hospice Social Worker will assess the psychosocial needs of patients and families while providing counseling and crisis intervention. They will also serve as a liaison between the patient and community agencies while participating in interdisciplinary team meetings to develop care plans.
Requirements
Candidates must hold a master's degree in social work, though a bachelor's degree in social work is also accepted. A minimum of two years of documented supervised experience in health care or hospice is required.
