Social worker

Key Responsibilities

The social worker will assess the psychosocial status of hospice patients and their families to provide counseling, crisis intervention, and support. They will also serve as a liaison between families and community agencies while participating in interdisciplinary care planning.

Requirements

Candidates must hold a master's degree in social work, though a bachelor's degree in social work is also accepted. A minimum of two years of documented supervised experience in health care is required, with hospice experience preferred.
