Key Responsibilities

The Hospice Social Worker assesses the psychosocial needs of patients and families while providing counseling and crisis intervention. They also collaborate with the interdisciplinary team to develop and implement individualized plans of care.

Requirements

Candidates must hold a master's degree in social work, though a bachelor's degree is also accepted. A minimum of two years of documented supervised experience in health care or hospice is required.
