Work Schedule

This position offers weekends off, day shifts only, a signing bonus. Work with UnitedHealth Group in Moore, Oklahoma, United States.
Key Responsibilities
Conduct comprehensive in-home assessments and manage patients through acute medical situations or chronic condition exacerbations. Collaborate with an interdisciplinary team to provide clinical care and advanced care planning in the patient's place of residence.
Requirements
Requires active ANCC/AANP certification for NPs or NCCPA certification for PAs, along with an unrestricted Oklahoma license and driver's license. Candidates should have the ability to travel daily to patient homes and possess strong clinical skills in geriatric or home health settings.
