On Call / Triage Registered Nurse Full Time Nights

Key Responsibilities

The Registered Nurse is responsible for planning and delivering skilled nursing care, including triage and in-person visits for hospice and palliative patients. They must coordinate with the interdisciplinary team, document patient progress, and provide education and support to patients and their families.

Requirements

Candidates must hold an active Florida RN license, a valid driver's license, and BLS certification, with a minimum of two years of acute care experience. Certification as a Hospice and Palliative Registered Nurse (CHPN) is required within 12 months of hire.
