Key Responsibilities
The Registered Nurse Case Manager oversees the patient's plan of care, coordinates multi-disciplinary services, and provides direct skilled nursing care in the home. They are responsible for accurate clinical documentation, patient education, and supervising nurse aides and LPNs.
Requirements
Candidates must have a current Registered Nurse license and at least one year of experience in an acute care setting. A valid driver's license and auto insurance are required, with home care experience preferred.
