Job Description

We’re building a world of health around every individual — shaping a more connected, convenient and compassionate health experience. At CVS Health®, you’ll be surrounded by passionate colleagues who care deeply, innovate with purpose, hold ourselves accountable and prioritize safety and quality in everything we do. Join us and be part of something bigger – helping to simplify health care one person, one family and one community at a time.

Do you have a passion for engaging with people and helping them on their journey to better health? Discover a flexible, meaningful role with the opportunity to connect, educate, and close gaps in care, emphasizing keeping people safe and well at home.

Signify Health, part of CVS Health®, is hiring per diem physicians (MD/DO) to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. These clinicians will work with us during peak demand months and/or in high-demand areas, depending on business needs.

In this role, you will:

  • Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area
  • Primary coverage areas include but are not limited to: Cambridge MA and surrounding counties.
  • Provide availability to complete in-home visits in areas of business need and/or during seasonal spikes in demand for visits

Execute a Strong Visit Completion Rate

Have the option to provide availability Monday - Sunday, with the majority of visits scheduled between 8 a. m. and 7 p. m.

Other Role Expectations and Requirements

Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)

Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics

Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)

  • Maintain all required supplies and associated diagnostic equipment related to DPS offerings
  • Maintain active medical licenses (MD/DO) in coverage area(s)
  • Maintain multi-state licenses (preferred)
  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
  • Be flexible to travel locally and within licensed states, as needed (up to 25%)

Willingness to execute a collaborating physician agreement, partnering with and overseeing Signify Health-contracted nurse practitioners or physician assistants and fulfilling all state-by-state requirements (preferred)

Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), required

Accommodate field observations and/or “ridealong” experiences, as requested

During our In-Home Health Evaluations (and other associated follow-up visits), you’ll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.

The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i. e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Part-time Employees Qualify For

  • A competitive pay structure aligned with number of visits completed (additional incentives may also apply)
  • Malpractice insurance coverage (during Signify Health-related activities)
  • Supplies and other perks
  • Why Physicians Enjoy Working with Signify Health
  • “Many of the people I see are shocked that a physician is coming to their home to visit with them one-on-one. When you sit down and have an unhurried conversation about their health, they are impressed, happy, and understand that their health plan and care team cares about them.” - M.L., MD

“The primary care physician gets a snapshot of a person in an office setting, but with Signify Health, we see things like the number of steps to enter their home, the state of disrepair those steps may be in, and the person’s ability to function comfortably and with ease in their own home. We can make referrals to the individual’s health plan, encourage the participation of their loved ones, and ensure their PCPs are aware of their health and overall quality of life.” - C.H., MD
