Your day to day as a RN Case Manager

As a PRN RN Case Manager at Avardis, you will take the lead in shaping best practice initiatives that enhance health outcomes, streamline access to care, and lower costs for the population we serve.

Your role includes providing clinical leadership for Managed Care, Bundle Payment, and ACO programs, acting as an educator and advocate for both patients and staff.

You'll collaborate across departments to evaluate and direct activities that support patient care delivery, oversee data collection for quality indicators, and identify successes and areas for improvement.

By developing comprehensive care plans with input from patients, families, and healthcare teams, you ensure smooth transitions from inpatient care to the next level.

To thrive as a PRN RN Case Manager with Avardis, you'll need a robust blend of skills and qualifications. A valid Degree in Nursing as a Registered Nurse is a must, along with at least two years of experience in a hospital, nursing care facility, or similar healthcare setting.

Comprehensive knowledge of managed care rules, regulations, and guidelines is essential, empowering you to navigate the complexities of health insurance and case management with confidence. You'll exhibit strong negotiation skills to ensure coverage and timely completion of case management reports.

Experience with a Health Plan or third-party case management company will give you an edge in this role.

Knowledge and Skills Required for the Position Are

• Must possess a Degree in Nursing as a Registered Nurse.
• Must have as a minimum two (2) years' experience in a hospital, nursing care facility, or other related health care facility.
• Must have a working knowledge of managed care rules, regulations, and guidelines.
• Must have comprehensive knowledge of managed care, health insurance, and Case Management.
• Ability to negotiate coverage and provide complete and timely case management, bundled payment, and ACO reports.
• Prior experience with a Health Plan, private or third party case management company preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
