Monday to Thursday 8:30 am to 4:00 pm – Friday 8:30 am to 2:30 pm.

Additional hours may be needed.

I Have A Chance Support Services Ltd. is seeking a dedicated Clinical Social Worker to join our multidisciplinary team. In this role, you will provide comprehensive support to adults facing challenges such as addiction, behavioral issues, and trauma. Your expertise in clinical social work and commitment to evidence-based practices will be instrumental in helping individuals develop effective coping strategies and resilience, ultimately enhancing their overall well-being.

Role Overview

As a Clinical Social Worker, your responsibilities will include conducting thorough assessments to evaluate clients' mental health needs, developing and implementing individualized treatment plans, and providing individual, group, and family therapy sessions.

You will address a spectrum of behavioral and mental health concerns, including substance abuse, compulsive behaviors, emotional dysregulation, anxiety, depression, PTSD, and other mood disorders. Collaboration with other healthcare professionals is essential to ensure comprehensive care for each client.

Key Responsibilities

Assessment & Treatment Planning

• Conduct comprehensive psychosocial and mental health assessments
• Develop, implement, and regularly review individualized treatment plans
• Assess and respond to complex mental health concerns including addiction, trauma, anxiety, depression, PTSD, and mood disorders
• Monitor client progress and adjust treatment plans as needed

Clinical Intervention & Therapeutic Support

• Provide individual, group, and family therapy using evidence-based practices
• Apply modalities such as Cognitive Behavioural Therapy (CBT), trauma-informed care, and motivational interviewing
• Support clients in developing coping strategies, emotional regulation, and resilience
• Maintain professional boundaries and a strengths-based, client-centered approach

Case Management & Care Coordination

• Coordinate care with multidisciplinary team members
• Participate in case conferences and interdisciplinary meetings
• Advocate for clients and facilitate access to community resources
• Promote continuity of care through effective collaboration

Documentation & Compliance

• Maintain accurate, timely, and confidential documentation
• Ensure compliance with ethical, legal, and organizational standards
• Utilize Electronic Health Record (EHR) systems effectively
• Adhere to privacy and confidentiality policies

Professional Practice & Ethics

• Practice in a professional, ethical, and culturally responsive manner
• Provide inclusive, nonjudgmental care
• Uphold professional standards and scope of practice
• Maintain appropriate therapeutic boundaries

Collaboration & Program Participation

• Participate in team meetings, case reviews, and quality initiatives
• Contribute to program development and best practices
• Share clinical insights to support holistic service delivery

• Engage in ongoing supervision and training
• Stay current with research and best practices
• Participate in accreditation, audits, and organizational initiatives

Working Conditions

• Regular interaction with clients experiencing mental health challenges, addiction, trauma, and behavioral issues
• Combination of office-based duties and direct client engagement, including individual, group, and family sessions
• Fast-paced and dynamic environment requiring adaptability and strong clinical judgment
• May involve managing crisis situations and responding to urgent client needs in a calm and professional manner
• Collaboration with a multidisciplinary team, requiring frequent communication and participation in meetings and case conferences
• Extended periods of sitting for documentation and administrative tasks using Electronic Health Record (EHR) systems
• Occasional requirement to work additional hours based on client needs or organizational demands
• Adherence to strict confidentiality, ethical standards, and professional boundaries at all times

Qualifications

• A master’s degree in social work (MSW) from an accredited institution is essential.
• Possession of relevant licensure as a Clinical Social Worker is required.
• A minimum of two years of supervised clinical experience in mental health or healthcare settings is preferred.
• Strong communication and interpersonal skills are essential for building therapeutic relationships with clients.
• Additionally, proficiency in conducting assessments, developing treatment plans, and providing various therapeutic interventions is crucial.
• Familiarity with electronic health record (EHR) systems is an asset.
• A commitment to ongoing professional development and staying updated with the latest therapeutic approaches and techniques.
