Hospice Field RN / Case Manager

About the Role

The Hospice Field Registered Nurse (RN) Case Manager plays a vital role in
providing compassionate, patient-centered end-of-life care in the comfort of
patients' homes and care facilities. In this role, you'll coordinate and manage
each patient's plan of care while focusing on comfort, dignity, and quality of
life. Working alongside an interdisciplinary hospice team, you'll provide
skilled nursing care, symptom management, patient and family education, and
emotional support during one of life's most meaningful journeys.

As a Hospice RN Case Manager, You Will

* Complete comprehensive nursing assessments and ongoing evaluations of hospice
patients.
* Develop, implement, and manage individualized hospice plans of care in
collaboration with physicians and the interdisciplinary team.
* Provide skilled nursing care focused on pain and symptom management, comfort,
and quality of life.
* Educate patients, families, and caregivers on disease progression,
medications, symptom management, and end-of-life care.
* Coordinate services with physicians, social workers, chaplains, hospice
aides, and other healthcare professionals.
* Monitor patient conditions and promptly communicate changes to the hospice
physician and care team.
* Supervise Hospice Aides and LPN/LVNs to ensure high-quality patient care.
* Maintain accurate, timely clinical documentation in accordance with Medicare
Conditions of Participation and hospice regulations.
* Participate in interdisciplinary team meetings and contribute to ongoing
patient care planning.
* Provide compassionate support and guidance to patients and families
throughout the hospice journey.
* Ensure compliance with all federal, state, and agency policies and standards.

Requirements

* Current, unrestricted RN license in the applicable state
* Current CPR certification
* Valid driver's license and reliable transportation with current auto
insurance
* Minimum of 2 years of nursing experience (Hospice or Home Health experience
preferred)
* Hospice Case Management experience preferred
* HomeCare HomeBase experience preferred
* Excellent assessment, communication, and time management skills
* Ability to work independently while collaborating with an interdisciplinary
team
