Inova Health Center 8081 – Fairfax East is looking for a dedicated Registered
Nurse (RN) Patient Care Navigator - Structural Heart to join the team. This role
will be Full-Time; Monday – Friday, No Holidays, No Weekends. Relocation
assistance eligible.

Registered Nurse (RN) Patient Care Navigator - Structural Heart Job
Responsibilities:
* Coordinates a patient's plan of care which may include appointment
scheduling, confirming patient's attendance, transportation, assisting with
referral process, and answering patient questions.
* Acts as a liaison, advocate and point of contact to patients in need of
services such as screening, diagnostic treatment, clinical research
participation, support services, and follow-up/aftercare plan by helping them
access the best healthcare outcomes from doctors, hospitals and others who
provide their required services.
* Refers cases to the appropriate resources for assistance. Anticipates
patient's needs and advocates for patient rights and appropriate utilization
of services.
* Conducts comprehensive assessment of the patient's health and psychosocial
needs and develops a case management plan in collaboration with the patient
and their support system.
* Educates patient, family or caregiver(s), and members of the health care
delivery team regarding treatment options, community resources, insurance
benefits, self-care management, etc. to facilitate timely and informed
decision-making throughout all phases of the continuum process.
* Collaborates with multidisciplinary teams to address patient questions and
responses about the care process and treatment options.
* Develops/maintains reliable systems to document, track and monitor patient
data.
* Performs other duties as assigned.
Minimum Qualifications
* Education: Bachelors Degree in Nursing
* Experience: 3 years of nursing experience
* Certification: Licensed in the Commonwealth of Virginia as a Registered
Nurse; Basic Life Support from the American Heart Association
Preferred Qualifications
* Experience: cardiac experience; familiarity with cardiac medications

Requirements
Candidates must hold a Bachelor's degree in Nursing and possess at least 3 years of nursing experience. A valid Virginia Registered Nurse license and Basic Life Support certification are required.
