Key Responsibilities

The Nurse Practitioner will conduct virtual clinical intake assessments and evaluate patient readiness for the Home-Based Pulmonary Rehabilitation program. They will also contribute to quality assurance, compliance activities, and the implementation of medical informatics tools to support clinical monitoring.

Requirements

Candidates must hold a Master’s or doctoral degree from an accredited Nurse Practitioner program and possess current, unrestricted APRN licensure, including California. A minimum of five years of progressive clinical experience as a Nurse Practitioner is required, along with relevant experience in pulmonary or chronic disease management.
