Key Responsibilities
The Registered Nurse Case Manager plans, organizes, and directs home care services while coordinating patient care for an assigned caseload. They perform comprehensive physical and psychosocial assessments, develop and implement care plans, and provide clinical direction to support staff.
Requirements
Candidates must hold a current and valid nursing license and CPR certification. A bachelor's degree is preferred, along with at least one year of home health or hospice experience and one to two years of recent acute care experience.
