Key Responsibilities
Provide one-on-one physical therapy to older adults in their homes or senior living communities using the Geriatric House Calls model. Develop individualized plans of care, perform evaluations, and collaborate with an interdisciplinary team to improve patient mobility and independence.
Requirements
Must hold a valid Physical Therapy license or be eligible to obtain one in the state of practice. Requires a degree from an accredited Physical Therapy program, basic computer and EMR proficiency, and current CPR certification.
