Key Responsibilities
The Advanced Practice Clinician will conduct comprehensive patient assessments and provide clinical care to high-risk members in nursing homes and assisted living facilities. They will also collaborate with interdisciplinary teams to coordinate care and facilitate advanced illness planning conversations.
Requirements
Candidates must hold an active ANCC or AANP certification for NPs or NCCPA certification for PAs, along with an unrestricted state license and driver's license. At least one year of clinical experience in a long-term care or geriatric setting is preferred.
