Key Responsibilities
The Advanced Practice Clinician will conduct comprehensive patient assessments, including physical exams and medication reconciliation, within nursing homes and assisted living facilities. They will also collaborate with an interdisciplinary team to provide advanced illness planning and ongoing clinical support for high-risk members.
Requirements
Candidates must hold an active ANCC or AANP certification for NPs or NCCPA certification for PAs, along with an unrestricted state license and driver's license. At least one year of clinical experience in a long-term care or geriatric setting is preferred.
