West Coast Remote Nurse Practitioner - CA, WA or AZ License Preferred

Key Responsibilities

The Nurse Practitioner will serve as the primary clinical point of contact for patients, managing remote care journeys and making clinical decisions based on evidence-based guidelines. Responsibilities include medication titration, clinical escalation management, and close collaboration with physicians and care teams to optimize patient treatment plans.

Requirements

Candidates must hold an active Compact Multi-State RN license and an active ANCC or AANP certification, with a preference for NP licensure in CA, WA, or AZ. The role requires at least 5 years of experience treating chronic diseases and 3 years of cardiology experience with a focus on heart failure management.
